The necessary documents listed (Please Scan All Documents, Any Document Not Scanned Will Be Rejected):

1. Current address in the US

2. Address in Jamaica or your Home Country

3. Were you ever on the J1 program? If yes, please submit copies of your J1 visas.

4. Clear copies of any H2B visas that you have.

5. A list of any dates that you were ever on the H2B program.

6. What was the most recent arrival date to the US for the H2B program?

7. Copy of your most recent I-94

8. Copy of any I 797’s that you have including the current one

9. A copy of your passport

10. Most recent 3 pay-stubs (good clear copies with nothing cut off).

11.  Copy of Social Security Card
